TERMS & CONDITIONS
 
  By completing the Frequent Guest Rewards enrollment card you agree to the terms and conditions of Frequent Guest Rewards. Terms and conditions may change at any time.

1. You must be 18 years of age to enroll.
2. The Frequent Guest Rewards Program is for individual customers only. Points may not be credited to business entities.
3. The $15 credit received on your second visit to any of the restaurants will not be given if your $15 membership fee has been waived for any reason.
4. Gift cards that are lost, stolen or expired cannot be replaced.
5. Only the member paying with a registered credit or debit card may earn points.
6. Points are earned for purchases of food, beverage, gift cards, tax and gratuity. Deposits are not eligible for points.
7. One point is earned for every $1.00 spent.
8. Points have no cash value. Points may be redeemed for gift cards and a variety of other rewards listed under POINTS on the Frequent Guest Rewards web site at memberadvantages.com.
9. Points are not transferable.
10. Cash transactions do not earn points.
11. Purchases prior to the date of enrollment do not earn points.
12. If questions arise, call 866.379.1281 or email  within seven days of the error.
13. Points must be awarded at the time of purchase. Points will show online after your second visit to a Fifth Group Restaurants location where you used your registered credit or debit card.
14. To qualify for points on party functions, you must be an active Frequent Guest Rewards member before booking the party (points for parties are given on food and beverage only).
15. Dollars spent with Bold American Catering are not eligible for the program.
16. Frequent Guest Rewards Dining Cards are not valid for payments toward events with Bold American Catering.
17. If a customer's account is inactive for one year, the account will be closed.
18. Points expire one year from the date of inactivity if not redeemed. If status is returned to active within one year from being made inactive, existing points will be reinstated.
19. Additional credit or debit cards may be added online at memberadvantages.com.
20. There may be certain dates where purchases do not earn points. You will be notified in advance.
21. Frequent Guest Reward Program rewards are not valid in conjunction with any other offers, discounts or promotional certificates.
22. Participating sponsors may impose their own rules.
23. Fifth Group Restaurants is not liable for any Federal or State taxes incurred due to Frequent Guest Rewards.
24. Gratuity for special events rewards are the responsibility Frequent Guest Rewards Program member.
25. Fifth Group Restaurants reserves the right to add or withdraw participating restaurants, to make changes in the rules or prizes, audit or cancel your account, or terminate the program at any time without advance notice.
